You can view and print a copy of your vaccination receipt by visiting the Ontario Vaccination Portal at https://covid19.ontariohealth.ca/ You must have a green photo health card to view your vaccine record online.

Pandemic best practices
Stay home when you are sick, even if you have mild symptoms
Wash your hands often with soap and water for 20 seconds or use hand sanitizer
Avoid touching eyes, nose or mouth with unwashed hands
Avoid sharing of personal items
Symptoms
Most common symptoms that require immediate self-isolation :
Fever (temperature of 37.8°C or greater)
New or worsening cough
Shortness of breath (dyspnea)
Decrease or loss of smell or taste
Fatigue, lethargy, general unwell feeling, or muscle aches and pains (for adults over 18)
Nausea, vomiting or diarrhea (for children under 18)
Less common symptoms and require self isolating:
Sore throat
Runny or stuffy nose
Abdominal pain (that is persistent and ongoing)
Headache (new and persistent, unusual, unexplained, or long-lasting)
Conjunctivitis (pink eye)
Decreased appetite
